It is primarily indicated for treating Diabetes Mellitus type 2 in people who are unable to respond to a monotherapy with either of the antidiabetic agent.

Maximum Dose: Metformin dose not to be exceeded 2000mg per day. Sitagliptin dose not to be exceeded 100mg per day for both therapies

Patients on metformin alone should take sitagliptin 100mg/day with prescribed dose of metformin. Patients alone on sitagliptin therapy should take metformin 1000mg per day plus 100mg per day of sitagliptin.
Study found it to be inappropriate to be used in hepatic impairments
In patients with eGFR (glomerular filteration rates less than30ml/min/1.73m2 it is contraindicated. However, in patients eGFR 30-45ml/min/1.73m2 the drug is not recommended.
Research on animals showed no effect and no human research was conducted. Or research on animals have shown some degree of effect but human research did not show any effects.
Study insufficient to conclude whether drugs enter breast milk, so the patients must use with caution.
In patients who have reported angioedema should be closely monitered. As per studies, in metabolic acidosis and diabetic ketoacidosis patients close care of therapy is recommended. Evidences of acute pancreatitis with hemorrhage have been reported. In elderly patients with age greater than 80 years close monitoring of therapy is recommended as per studies with inadequate renal functioning.
Monotherapy: This medicine is used for treating type 2 diabetes (condition characterized by elevated glucose levels) that is inadequately controlled by metformin, diet and exercise.
Dual therapy: It is used for treating diabetes type 2 along with metformin or a glitazone when patients respond inadequately to diet changes, exercise and either metformin or glitazine, or in combination with sulphonylureas in patients contraindicated for metformin, and for patients that do not respond adequately to sulphonylureas alone.
Triple therapy: This medicine is used with metformin and a sulphonylureas or glitazone for treating diabetes 2 that is inadequately controlled by dual therapy.
With insulin: This medicine is used with or without metformin for treating type 2 diabetes in patients who respond inadequately to diet, exercise, and insulin.
The patients administered with this medicine may experience pancreatitis (inflammation of pancreas), allergic reactions, Stevens-Jhonson Syndrome (allergic reaction), Hypoglycemia (low glucose levels), bullous pemphigoid (large fluid-filled blisters), osteoarthritis (weak bones), upper respiratory tract infections, flatulence, swelling, and influenza (flu-like symptoms).
The patients with diabetic ketoacidosis (condition causing fat breakdown at an abnormal rate) and moderate to severe renal (kidney) impairment should avoid use of this medicine.
This medicine should be taken with food.
This medicine is indicated in maturity onset diabetes either alone or with sulphonylureas. It is also used as adjuvant therapy in insulin dependent diabetes, particularly in obese patients.
GI upset, taste disturbance, abnormal liver function tests, hepatitis (inflammatory condition of liver), skin reactions, reduced B12 absorption are some of its side effects. Rarely it may cause lactic acidosis (buildup of lactate causing acidic pH of blood stream).
This medicine is contraindicated in patients with diabetic ketoacidosis (life threatening condition in which liver breaks down fats into ketones for energy) or precoma (a state of near unconsciousness), renal (kidney) or hepatic (liver) impairment, acute conditions predisposing to renal impairment, cardiac or respiratory failure, recent myocardial infarction (heart attack), alcoholism and in patients with administration of iodinated contrast materials.

Inosita Plus is an oral anti-hyperglycemic medicine which is a combination of 2 drugs; sitagliptin and metformin.
Sitagliptin works by DPP-4 inhibition, which is supposed to exert its activity in patients with type 2 diabetes, by reducing the inactivation of incretin hormones. Concentrations of the active complete hormones are amplified, thereby increasing and lengthening the action of these hormones. Incretin hormones, including GLP-1 and GIP, are secreted by the intestine throughout the day, and levels are increased in response to a meal. These hormones are quickly inhibited by the enzyme, DPP-4. By modifying active incretin levels, it increases release of insulin lowers glucagon levels in the circulation, depending on glucose levels.
Metformin works by increasing glucose tolerance among patients with T2 diabetes, and by reducing both basal and postprandial glucose in plasma. It also lowers hepatic glucose formation, decreases intestinal uptake and increases insulin sensitivity by raising peripheral glucose uptake and consumption.Insulin release remains unaffected while fasting levels and 12-hour plasma insulin levels may decrease.
